About the Practice
We are live in Washington and Arizona, with Florida planned for 2027. If you are outside these states, join the Blueprint list and we will notify you when we are live near you.
You are assigned a board-certified Nurse Practitioner from our clinical team for your entire journey — same provider every visit, every message. No rotating care teams. No delegated messaging. Your specific provider and their full credentials are disclosed once you complete intake.

Pricing & Payment
Membership is $295/month and includes comprehensive labs at month one and every 3 months, monthly telehealth visits, GLP-1 prescribing, muscle preservation guidance, and structured off-ramp planning at no additional cost. Medication is billed separately at compounding pharmacy cost.
The Elysiv membership fee is billed monthly and not run through insurance. Prescriptions written by your provider can be filled at any pharmacy and processed through your insurance like any other prescription. Lab work can also be billed through insurance separately. HSA/FSA accepted for the membership fee.
Yes. No contracts, no minimum terms. If you decide to stop, we will also help you taper and build a maintenance plan so you do not regain what you lost.
Medication & Care
We prescribe what is clinically appropriate and available. Brand-name (Wegovy, Zepbound) is our default when your insurance or budget supports it. Compounded alternatives are available through vetted partner pharmacies when appropriate.
Yes. Many patients come from other telehealth companies looking for a real clinical relationship. Your intake will capture your current regimen and we will transition care seamlessly.
At minimum: comprehensive metabolic panel, lipid panel, HbA1c, thyroid panel (TSH, free T4), vitamin D. Depending on your case we add hormones, inflammation markers, and nutrient status. Ordered to Quest or LabCorp near you.
Message your provider. We adjust your plan when symptoms warrant it. For anything serious — severe abdominal pain, persistent vomiting, signs of dehydration — call your provider or go to the ER. The Companion Blueprint has a full triage guide.
Privacy
The intake asks about your goals, health history, and contact details. Health answers (conditions, medications, height, weight) stay in your browser — they never leave your device until you book a visit with a provider. Only your name, email, and state are used for lead follow-up.
Clinical visits, messages, records, labs, and prescriptions are delivered through HIPAA-eligible vendors. This marketing site does not store protected health information — intake health answers stay on your device until you book a visit with a provider. As we onboard each clinical vendor, we execute the corresponding business associate agreement (BAA) before any PHI flows through it.
